Subject Access Request Forms

Subject access request forms must be submitted in paper form to Hampshire County Council's data protection officer, as signatures and evidence of identification are required. You may print your own form, if you have a printer, as an alternative to asking for a form to be sent to you by post. An additional option is to collect a form from one of Hampshire County Council's Information Centres.

Please note that if you require a police records clearance check, then you will need to contact Hampshire Constabulary directly.
