Survey forms
The School Travel Planning Team have produced a selection of travel surveys that will enable you to consult widely with your school community, which is a DfE requirement. They are available in a PDF version, a Word version (so that you may personalize it to your school's requirements), or you may request copies from the School Travel Planning Team.

Parents and Pupils (KS1 and 2)
- this can be taken home by the pupils and filled out with their
parents.

Students (KS3 and 4) - can be ideally filled out during
tutorial time. Parents can be asked to send in any comments if they
wish to contribute.

residents within the schools' community to gain feedback on any
impacts felt by residents of the journeys to and from the school.
