Has your setting worked with the child or young person and the parent/carer to deliver SEN Support as part of a "graduated approach"?
A graduated approach is a four part cycle - assess, plan, do, review.
You and the parents/carers can decide what worked and what didn't in that cycle. Then you can change the plan as needed to help the child or young person make progress.
You will need to show evidence of SEN Support as part of the graduated approach as part of the EHC assessment request.
Yes, we have supported the child or young person in this way
No, we have not delivered SEN Support as part of the graduated approach
You will need to use the graduated approach to show a child or young person's need for an EHC assessment.
Further information about the graduated approach is available here: SEN Support and the graduated approach.
If you can't show you have given SEN support as part of the graduated approach, your request may not be able to proceed.
You can choose to continue this checklist anyway.
