What you need to do now

1. Based on your answers in the checklist, decide whether to make the request.

If the checklist advised that your request may not be able to proceed, talk to the parents/carers and the Hampshire Special Educational Needs (SEN) Service to agree a way forward.

2. If you decide to proceed with the request, talk to the parents/carers. They must be aware that you are making the request and why.

3. If, after reading all the advice given, you would like to make the request, you may now proceed.

If you have requested EHC assessment through the EHC Hub before and have an account, you can log in to request an EHC assessment.

If you have not requested EHC assessment through the EHC Hub before, you need to create an account to request an EHC assessment.
