CARE MANAGEMENT

1 The Executive Member was pleased to authorise the implementation of the outcome of the Best Value Review of Care Management.

2 A key point that arose from the review was the creation of a Care Brokerage Service which, in common with Financial Assessment and Benefit Service, will free up valuable Care Manager time and enable a more efficient service for everyone.

3 The Care Broker role involves:-

    · Establishing resource availability.

    · Planning and negotiating care arrangements.

    · Raising relevant orders.

    · Making minor variations to care packages within agreed limits.

    · Generating and sending out care plans.

    · Liaison with users and carers.

    · Gathering and maintaining cumulative information on the performance of providers and profiling market defects.

4 Trials of the Care Brokerage System in the pilot area (Eastleigh) had delivered qualitative and quantative gains for stakeholders. These included:-

    · A reduction in the waiting time for care management assessments leading to an improved service to users and other agencies.

    · A reduction in the number of outstanding scheduled client reviews which were now undertaken by reception and assessment care managers.

    · A substantial improvement in job satisfaction ratings expressed by care managers.
