Malmesbury Lawn Home for Older People - Upgrade of Home and

provision of Intermediate Care Beds

Summary

1.1

At the November 2002 meeting of the Executive Member - Social Care a report by the Director of Social Services outlining the future proposals for Malmesbury Lawn was agreed. The report gave approval in principle to the reorganisation and upgrade of the home for older people to provide residential care to Elderly Mentally Infirm (EMI) service users and a self contained six bed Intermediate Care Unit (ICU). These plans were approved by the National Care Standards Commission on the 14 October 2002.

The upgrade of the home forms part of the final phase of the refurbishment work in the Older Persons Homes Reinvestment Strategy. This, together with funding secured as part of the Government initiative for Intermediate Care along with an allocation from the Performance Fund, a scheme to provide a high quality residential care service, including IC beds for EMI service users will be provided at Malmesbury Lawn.

Funding for these proposals will come primarily from the Older Persons Homes Reinvestment Strategy. Additional funding will be provided from two government grants including the Intermediate Care Cash for Change fund and the Social Services Performance fund.

The purpose of this report is to outline the proposals and seek approval to the attached building design project appraisal.

2.

Scope of the Works

2.1

The existing residential building is of a quadrangle design with an inner enclosed courtyard area. The new Day Centre constructed in 2001 joins the southern elevation of the main building via an enclosed link. The proposals to upgrade the building will be achieved by reorganising and reordering the existing accommodation. No new accommodation will be constructed.

2.2

The reorganised accommodation will provide an Intermediate Care Unit (ICU) on the ground floor along with residential accommodation, staff, ancillary and kitchen facilities on the ground and first floors. The ICU accommodation will comprise six bedrooms, assisted bathroom, assisted WC, one staff WC, living room, dining room, kitchenette and therapy room.

2.3

The remainder of the ground floor will accommodate the following residential accommodation: 13 EMI bedrooms, two assisted bathrooms, three assisted WCs, communal living/dining room, stores and sluice room. The existing staff accommodation and administration suite, which will remain unaltered, will comprise: reception/duty office, administration office, manager's office and medical room. A staff and visitors room will be provided by the adaptation of an existing WC and store. The existing main entrance to the building, which is adjacent to the administration suite, will be slightly enlarged to provide a larger lobby area. New double security doors will be fitted to the main entrance and will provide a dual access point to the EMI accommodation and ICU.

As part of this scheme the kitchen on the ground floor will be reorganised and refurbished. Since the construction of the Day Centre the existing kitchen facilities have temporarily been able to cope with the production of additional meals in a kitchen which was not upgraded in line with the rise in meal production. It is therefore proposed to reorganise and redecorate the kitchen and repair or replace existing fittings and equipment. The reorganisation will be carried out to address health and safety issues and include improved ventilation. Further details are included in the attached design project appraisal.

2.5

The first floor accommodation will be dedicated to residential accommodation and communal facilities for EMI service users along with a small area for staff quarters. The accommodation provided will comprise: 18 EMI bedrooms, two assisted bathrooms, five assisted WCs, communal living/dining room, kitchenette, smokers room, hair dressing salon and sluice room. The staff sleep room and en suite will also be located on the first floor.

Along with the internal reordering of the building to provide adequate sized bedrooms, communal areas, WC and bathing facilities for the ICU and EMI service users the building will also undergo a complete overhaul. This work will include a complete re-wire with lighting updated as necessary; installation of a new fire alarm system and security system; replacement of all internal fire doors with new electronically operated, fire resistant doors. The existing heating system is the original system installed in the building some thirty years ago and is now at the end of its life expectancy and is considered to be uneconomical to repair, therefore, a new heating system from the existing boilers, feeding low surface temperature radiators will be installed in the building. These electrical and mechanical works will be complemented by the complete redecoration of the building to include new floor finishes throughout, new suspended ceilings in the ground floor corridors and replacement of the ceiling void fire barriers, where necessary.

Phasing

3.1

Five residents have expressed a strong desire to remain in the home whilst the building works proceed. This has been agreed and plans to re-accommodate up to 18 of the residents at Green Meadows are in hand. This follows the recent completion of the refurbishment works at Green Meadows as part of an earlier phase of work in the Older Persons Home Reinvestment Strategy. A dedicated care manager for the scheme will be appointed to cater for the accommodation requirements of other residents during the proposed works.

4.

Funding and Timing

4.1

Funding for these proposals will come primarily from the Older Persons Homes Reinvestment Strategy, with £1,150,000, excluding fees, being allocated from the remainder of the Strategy funding. Two government grants will be combined with this funding and will include £259,000, excluding fees, from the Intermediate Care Cash for Change fund and £150,000, excluding fees, from the Social Services Performance fund. This provides a total budget of £1,559,000, excluding fees.

4.2

Although the latest revised costing for the proposed scheme at Malmesbury Lawn is greater than originally budgeted, the scope of work has increased to include some highly serviced areas for a refurbished kitchen. Phasing of the work to allow the Home to remain open during the works is also placing a premium on the cost of the project. Nevertheless, the project links to the investment already made on the site for a new Day Services Centre and revitalises an existing Home for the next 10 to 20 years.

4.3

A budget allocation of £989,000, excluding fees, for the refurbishment of the residential accommodation was the amount included in the Older Persons Reinvestment Strategy budget. Since the initial design of this scheme there has been significant change in the commissioning needs for residential care with a focus on older people with dementia and the provision of intermediate care beds. Consequently the design has developed to reflect these latest changes and further funding has been added to the scheme. The project is now sufficiently defined to enable the scheme to be costed and for the attached design project appraisal to be prepared.

4.4

In order to accommodate the brief and take account of inflation an additional amount of £161,000, excluding fees, has been added from the Strategy funding. The Intermediate Cash for Change funding has been added to this scheme to realise the provision of the ICU. The Social Services Performance funding has been included in the scheme to fund the work being undertaken in the kitchen and to enable the phasing of the work in line with the recommendations from Social Services and requirements of the National Care Standards Commission.

4.5

It is anticipated that tenders will be invited during March 2003. Work will commence on site during June 2003 and be completed during June 2004.

Conclusion

These proposals will provide much improved facilities for the EMI service users of the Havant and will realise the completion of an ICU within the Home for Older People. Approval is accordingly sought to the attached building design project appraisal.
