Anyone whose Physical, Sensory, Mental or Emotional state means they are at immediate high risk of harm to themselves or others.

Anyone who is suffering serious abuse or neglect or where it is strongly suspected to be occurring.

Anyone who has no awareness of common dangers - who puts themselves at risk - is unable to make their needs known.

Anyone who is unable to maintain a safe environment

Anyone faced with immediate problems because a carer is no longer able/willing to continue in a caring role.

Anyone whose loss or lack of ability to perform activities of daily living mean they require immediate assistance to prevent harm.

Anyone whose social supports and relationships are at significant risk of breaking down.

Anyone who requires immediate assistance or management to prevent inappropriate interaction leading to harm to self or others.

Anyone whose independence is greatly reduced because they have significant physical, sensory, mental or emotional problems

Anyone where abuse or neglect has occurred and is having an effect on their current functioning.

Anyone where abuse is strongly suspected.

Anyone who has episodes of severe mental illness.

Anyone who shows little awareness of common dangers.

Anyone for whom there are strong indicators of inability to maintain a safe environment.

Anyone who needs to change accommodation to overcome major difficulty.

Anyone where the carer relationship is stressed and at risk of breakdown.

Anyone whose loss or lack of ability to perform activities of daily living indicates the need for assistance several times a day over a 24 hour period.

Anyone whose absence of occupational activity or social contacts is causing distress and deterioration.

Anyone whose social supports and relationships are at risk of breaking down

Anyone who requires regular or frequent assistance to minimise interaction with others which is likely to lead to harm to self or others....and/or compromises the ability to maintain community living.

Anyone whose Physical, Mental health or emotional state sometimes affects their ability to live independently.

Anyone whose physical, mental health and/or emotional state is not affecting their ability to maintain community living but who require assistance or monitoring to maintain independence.

Anyone who has awareness of risk but is not acting accordingly to maintain safety of themselves or others.

Anyone whose physical condition /sensory disability is not affecting their ability to maintain community living but which significantly reduces level of independence and quality of life.

Anyone where accommodation presents some difficulties indicating some risk to independence .

Anyone whose carer has occasional difficulties and who has limited support networks to call on.

Anyone whose lack or loss of ability to perform activities of daily living requires assistance at least once a day to maintain community living.

Anyone who has difficulty managing

some aspects of work, education, learning and/or social contact.

Anyone who requires assistance to develop social skills in order to maintain independence and achieve reasonable quality of life.

Anyone who has some difficulties in carrying out family responsibilities.

Anyone for whom absence of occupational activity or social contact is detrimental to a persons quality of life.

Anyone who is able to maintain safety of self and others and only requires advice or information to maintain safety.

Anyone whose physical / sensory disability either does not affect or only slightly affects their ability to maintain independence and community living.

Anyone whose carer has good support networks to call on.

Anyone who has difficulty with one or two aspects of daily living but who can make their needs known and arrange for appropriate help.

Anyone who has difficulty undertaking one or two aspects of work/learning/education / family life and/or social networks indicating little risk to independence.
