C28 - Intensive home care Performance Assessment Framework (PAF) indicator showing number of households receiving more than 10 hours of care over 6 or more visits
PSA - Public Service Agreement
C26 - PAF indicator showing admissions of supported residents aged 65 or over to residential/ nursing care
C32 - PAF indicator showing older people helped to live at home
DIS - Delivery and Improvement statement returned to Department of Health in Spring and Autumn showing planned and forecast performance over the financial year.
Intermediate Care - Short term (6 weeks or less) multi- disciplinary schemes either in residential or community setting to facilitate early discharge from hospital or prevent admission. Service users are not charged for Intermediate Care (IC)
PDT - Preventing Dependency Teams (see IC above)

NHS FNC - NHS Funded Nursing Care (formally known as `free' nursing care) 3 bands apply to people in nursing homes
FACS - Fair Access to Care Services - the new eligibility criteria whereby people's needs are assessed as either critical, substantial, moderate or low
