Podiatrists see all their diabetic patients annually for a risk assessment using a nationally recognised assessment tool. The patients are then seen according to their risk level. Practice nurses and districts nurses also use this assessment tool when reviewing diabetic patients, and a changed score would mean a change in care plan with referral as appropriate.

    3. Education sessions for newly diagnosed diabetics have recently commenced in the PCT. Sessions are available to all via their practice.

    4. Not routinely relevant in EHPCT, but accessible through diabetes centre

    5. All patients cared for by their GP practice are seen annually as a minimum. Patients cared for by the diabetes centre are seen approx every 15 months.

    6. The PCT does not have a protocol, but the diabetes centre has a young persons / adolescent clinic, and the PCT is working towards a cohesive care pathway for all diabetics which would include this aspect of care
