The project is being undertaken as the Council's contribution to the work of the Innovation Forum of local authorities awarded excellent status under the CPA. It involves working with health colleagues to promote the well-being of older people, reducing their need for emergency care - especially hospital care. Nurses and social workers allocated to primary care practices work proactively with older people at high risk. The key aims are to develop methodologies for identifying people at risk, assessing their circumstances, formulating care and contingency plans, mobilising a wide range of community resources to help them, and monitoring improvements in well being and savings to emergency services. Early results show that the project saves more than it costs and leads to major improvement in older people's well-being (although it is too early for firm conclusions).

The funds would be used to continue the project through 2005 / 06, to produce a full assessment of the methodology with supporting evidence, to prepare a bid for DH funds and formulate arrangements for `main streaming' the approach.

How does this bid support the targets within the Community Strategy?

The project supports Aim Number One of the Corporate Strategy (maximising life opportunities) by contributing to the Cabinet Priority for Older People. It also contributes to key targets in the National Service Framework and the second round of Local Public Service Agreements.

Overall funding arrangements

The project has an overall budget of about £150,000 in 2005 / 06. £50,000 is being contributed by the Social Services Department. Four PCTs are funding the salaries of nurses working on the project (to the tune of about a further £50,000). The project is being used to prepare a bid for funds from the Department of Health during 2005 to expand it and prepare it for full deployment across the County.
