The bid is for a contribution towards the funding of dedicated project management time to take forward the Oakridge Hall for All as a centre of excellence for older people. The vision of the project is to draw on local resources to establish a vibrant centre of excellence for older people in Oakridge that enhances their quality of life through promoting social, physical and mental well being. Phase 1 of the project established a number of themed events to run along side existing and new services for older people. A contribution of £3,500 from the Community Strategy budget was made in 2004/05 to support these activities, including the provision of transport and to contribute towards the cost of project management, along with Basingstoke and Deane Borough Council. There was a successful launch of the project in December 04, and the initiative was highlighted as part of the peer review in March 05 conducted by the European Trans-regional Network for Social Inclusion. It is now intended to develop the project to a second phase so that more older people are aware of its existence and where the local services can all link up to work together in the centre. Phase 2 will introduce a "Promoting Independence Programme." Specialist workers will be trained to cross the statutory and voluntary sector in assessing older peoples needs and mobilising the appropriate resources to maintain independence. This could mean increasing welfare benefits, helping people to find transport for visiting hospital, advice on housing support and safety. The project aims to secure external funding from sources such as the Hampshire bid under the "Partnerships for Older People's Projects" programme. This will enable the project to develop a centre within Oakridge where older people can learn how to use a range of assistive technologies which will further optimise their independence. To complete phase 1 of the project and to take it into phase 2 is is necessary to secure dedicated project management time. This has been estimated at £20,000 per annum. The Borough Council have given £5000 and have underwritten contributions from the Police and Housing. The project manager will be expected to revise the business plan of the project, to seek funding from outside sources (with the support of the Borough Council) to co-ordinate all work streams of the project, to problem solve as necessary, to provide ongoing evaluation of the project and to track progress towards achieving the end goals. The project manager will report to the LSP. The HCC/PCT Partnership Manager will hold the budget, provide project sponsorship and mentoring support. | |
