Section 5

    · If you have previously sent us a copy of your constitution and you have not amended it you need not send another copy.

    · If you have reviewed and/or changed this document you will need to send the new one and the minutes of the trustee meeting where it was reviewed/considered.

Section 6

    · You should have a budget for the current year and this should have been monitored at your trustee meetings. If you are unable to provide the budget or the minutes of the trustee meetings where it has been monitored please contact your Area Community Support Officer.

    · Your budget is in deficit if you are unable to cover your costs from the utilisation of your own assets (e.g.. your building, your trading company) without revenue grant aid.

    · Please list grants you have obtained from other sources including district and parish councils. Please say what the grants were for and if they are likely to be repeated in 2007/08.

    · A copy of your budget for 2007/08 is not required if you have not yet produced it. However, we do need to know if there are to be any changes to your budget such as an increase in staff costs, money being set aside for equipment, an increase in income from trading, etc.

    · Reserves. If you do not have a reserves policy please contact your Area Community Support Officer

10 Your activity programme -

Please discuss this section with your Area Community Support Officer who will assist you with this section

    · This is the activity programme of the Community Association only. Do not provide a list of activities that take place in your building that are outside the control of the trustees. If you hire your building to other organisations please list as `managing premises for a range of local groups under the `service and infrastructure provision column'.

Other examples

    · If you provide equipment for local groups or individuals (such as ICT equipment - learndirect facilities ) please list under 'Resourcing'.

    · If your organisation has sections please list their activities under the appropriate headings

    · If your trustees or staff attend local meetings on a regular basis ( such as the local community safety partnership ) please list under appropriate column.

    · If you provide meeting space for these meetings please list under Resourcing'

    · If you have held a consultation event please list under "Engagement and Representation"

This is not a definitive list - if there are other activities that your Community Association is involved with please include under the relevant heading.

11 New activities

Your project should help to achieve one of the outcomes below. Please list the appropriate outcome in column 1.

Outcomes

Maximising Well being by

    · Increasing participation through the development of activities for older people to support their continued well being

    · Increased participation through the development of activities for children and young people that will help them live positive lives

    · Increased participation through the development of physical activity programmes as part of the 30 minutes a day exercise regime

    · Increased access to information and services through ICT supported projects

    · Increased inclusion by extending activities to hard to reach, disadvantaged and black and minority ethnic groups

Column 2 --- you need describe the actual activity

e.g. by arranging for local children to have a day out in a country park,

    by providing one hour taster sessions in Yoga or dance fit at different times of day

If you are unsure what you could do ---- the Community Support Team have put together information about a range of activities which you could provide at your centre in order to meet one of the outcomes. Please see enclosed information.

Column3 --- You need to say how you intend to evaluate your activity

e.g what information are you going to collect to demonstrate you have increased participation.

12 Description of project

Describe your activity - include when it is going to take place, if the project is an expansion of some current activity or new, how many people you are expecting to participate.

13 Identified need

Include here information about local surveys you have carried out. If its something suggested in a local plan ( e.g. local strategic partnership, community safety partnership, parish plan etc)

14 Co ordinator

This will either be a trustee, sub committee or member of staff. List also the number of other volunteers who may be involved.

15 Evaluation

This needs to be a named person we can contact.

16 Budget

Please list your expenditure proposal. Spaces have been left for you to add other costs. If you are looking for funding from other sources for this project please let us know.

17 Sustainability Plan

In order to obtain this funding you need to demonstrate you are regularly reviewing and up dating your organisation's business processes. You should provide a minimum of 3 activities in total in this section. Types of activity are:

    · Publicity and marketing actions

    · Skills audit - developing a training plan for staff and volunteers

    · Review of staff appointment process

    · Developing a staff handbook

    · Developing volunteer policy and job descriptions for volunteers and trustees

    · Review of trading activity

    · External funding plan

4)

    · Reviewing health and safety processes

    · Reviewing policies

    · Reviewing staffing structure and needs

    · Develop a rolling maintenance programme
