1) The following decisions were made: |
1.1 Support was given for the recommended approach to this element of the Hampshire Model and the proposals for the delivery of aspects of Free Crisis Care, as well as for the extension of the Community Innovations Teams across the County. |
1.2 Approval was given to review the current respite and short term residential contracts with a view to ensuring there is the correct balance of traditional respite care and short term intensive re-ablement resources particularly for those people who are physically frail. |
1.3 Approval was given to commence detailed planning for the delivery of "Time to Think" beds across the County, during 2009/10; to be achieved via a mixture of in-house and independent sector provision and subject to a detailed financial analysis of costs and savings. |
1.4 Approval was given to invite tenders for a countywide Welcome Home contract with an implementation date in April 2010. |
2) Reasons for the decision |
2.1. The report of Hampshire County Council's Commission of Inquiry into personalisation and the future of adult social care, `Getting Personal: a fair deal for better care and support,' published in November 2008, included a recommendation that free care is available `for all those at risk of admission to hospital and in need of urgent social care.' This recommendation, along with measures to support people on discharge from hospital, has been included in the development of the Hampshire Model for adult social care. |
2.2. Improved outcomes for the residents of Hampshire County Council |
2.3. Improved efficiencies at the interface of health and social care provision particularly around performance on hospital discharge. |
3) Other options considered and rejected: |
3.1. Option 1: To make no changes to the service. |
a) Risk Assessment: No change would mean that the service would be unable to respond to changing service user expectations and to the increased demands of changes in demography in Hampshire as demonstrated by the Hampshire Commission. |
3.2. Option 2: To provide free care for all those first coming into contact with Adult Services at a time of any crisis |
a) Risk Assessment: This was rejected due to uncertainties around predicting demand and identified risk to the budget and service capacity to deliver |
3.3. Option 3: Fully integrated health and social care teams with Hampshire Community Health Care to create a health and social care model of crisis care. |
a) Risk Assessment: Whilst this is a long term objective major structural and workforce change in the short term would create significant barriers to short to medium term progress and improved outcomes for service users |
