IMPLEMENTATION OF THE HAMPSHIRE MODEL WORKSTREAMS

At her meeting held on 27 March 2009 the Executive Member received an update on the implementation of the Hampshire Model workstreams arising from the report of the Hampshire Commission of Inquiry `Getting Personal: a fair deal for better care and support'. The report submitted to the Executive Member also contained specific detail regarding the countywide roll out plan for Self Directed Support (SDS).

Adult Services is developing detailed governance arrangements and actively planning for the incremental implementation of the Hampshire Model. The department is managing the change in six linked workstreams, all of which are currently being worked on in detail to identify the required resources for successful delivery. The six workstreams are the Universal Offer, Free Crisis Care, Self Directed Support, Carers Initiatives, User Led Initiatives and Market Development.

The Universal Offer workstream is aimed at ensuring that older, disabled and vulnerable adults are able to access support within their community and universal public services so that they can access a full life. This involves working in partnership across the County Council, with District & Borough Councils and the voluntary sector to ensure that there is a range of ways that people can be active, engaged and safe in their communities.

The Free Crisis Care workstream is aimed at meeting the Hampshire Commission ambition of free care for all those at risk of admission to hospital and in need of urgent social care or those being discharged from hospital. A separate report refers. The Market Development workstream is aimed at ensuring that there is choice for people, whether they opt for a direct payment, a commissioned service, or a mix of the two.

Improving support to carers and the involvement of carers and service users in the design of services are fundamental to personalisation. Supporting unpaid carers will be key to our response to the ageing population The Hampshire Commission also demonstrated the power and benefits of carer and service user involvement in the design of services. The Hampshire Model aims to build on the high level of engagement achieved by the Hampshire Commission, to ensure that carers and services users shape the future of the services they use.

The Hampshire Model also builds on the current SDS project. Self Directed Support has been operating in Basingstoke running since May 2008 with the objectives of phase 1 being to (i) implement Self Directed Support in one district Adult Social Care office and (ii) develop a target operating model for the future of Adult Services in Hampshire. An internal review was carried out of this first SDS phase, interviewing staff, project team members and service users who were involved. The review concluded that the pilot had been successful but it also identified a number of learning points that will be helpful for wider implementation across the county.

Lessons from the SDS trial have proved that it is crucial for staff to be fully involved and engaged, with the appropriate systems in place to support them. As such, a two year development plan for county wide implementation has been designed. The first year prepares the ground for a smooth implementation and on successful completion of the first stage, Year 2 (commencing January 2010) will begin with focused training for frontline staff on the mechanics of delivering SDS. This will be delivered locally, to maximise coverage and minimise disruption. Following three months of focussed training, the locations trained will `go live' for all new service users, and for those coming up to review.

Training and `go live' will be phased geographically, in order to dedicate sufficient resource to on-site training and support. It is likely that this will commence in the North (January - April 2010), followed by the South (April - July 2010) and then the West (June - October 2010). However, this will need to be confirmed following more detailed planning and may therefore be subject to change. The proposed approach being undertaken will allow the department to meet Department of Health targets for Self Directed Support . The lessons learned from the phase 1 implementation in Basingstoke will also provide support for the staff, service users and partners involved while continuing to place Hampshire at the forefront of transformation to a more person-centred or `personalised' adult social care system.
