Reablement hub

Reablement occupational therapists (OTs) work with adults with identified needs (physical, social and psychological) enabling their participation in daily activities.

The role is diverse including assessment for hospital discharge; care needs and OT specific assessment for community-based clients. Providing functional assessment of clients, the OTs support them through aids and adaptations, use of compensatory techniques and reablement. This includes long-term housing needs, providing major adaptation and housing recommendations.

Short Term Bedded Services

Occupational therapists in Care and Support Services ensure positive outcomes for individuals who need additional support. The Short Term Bedded Services aim to achieve timely hospital discharges and prevent unnecessary admissions.

Supporting people for up to 28 days following hospital discharge, these recovery and assessment suites are in three of our nursing homes, working in partnership with the NHS.

Our occupational therapists are responsible for conducting assessments and delivering the required interventions. Whenever possible, community-based options are offered to residents.

Contact, Resolution and Assessment Team (CART)

Occupational therapists in CART work closely with social work professionals. Based in Fareham, the CART team serves as the first point of contact for individuals seeking advice and support from Adults’ Health and Care.

Our team collaborates closely with other social and health care professionals to manage incoming enquiries via telephone, online forms and webchat. They assess residents’ needs and provide them with the necessary support and guidance to maximise their wellbeing.
