Our dedicated and talented social care practitioners make a significant impact on the lives of those they care for. They provide essential support to Hampshire residents aged 18+ who may have disabilities, illnesses, or age-related challenges, helping them with daily tasks and improving their quality of life so that they have the maximum possible independence. Residential care roles

Our nursing homes have a team of Care Assistants that provide vital care to meet our residents’ needs. Wellbeing Co-ordinators plan and organise meaningful activities for our residents to enjoy. These roles along with others, such as General Assistants, Companions, Catering staff, Family Liaison, Receptionists and Administrators all play a key role in creating a welcoming, comfortable and safe environment the residents can call their home.

Our homes, respite and crisis units for people with learning disabilities are staffed by Senior Residential Services Officers and Residential Services Officers who support people to feel at home and enjoy their stay whether it is long or short term. Day services roles

Day Opportunities Coordinators and Day Opportunities Support Workers work together in our Day Services. Supporting people to enjoy different opportunities and activities in the local community.

Community care roles

Community Reablement Assistants  work directly with individuals to help them regain or develop the skills needed to live independently - whether following a hospital stay or a sudden change in circumstances that may affect their ability to remain at home. They provide short-term, personalised support in people’s homes, helping individuals build confidence, learn new skills and maintain independence within their community. They also work closely with families and support networks to upskill and enable them to continue providing care. We have Level 1 and Level 2 Community Reablement Assistants and Reablement Practitioners who work across all disciplines within the Reablement service including sensory loss, occupational therapy and the community response team (CRT). Leadership roles include our Reablement Team Leaders.

Our wider reablement team includes Occupational Therapists, who carry out functional assessments and Sensory Services Officers, who offer specialist support for individuals with hearing, sight loss or other sensory needs. We also work in close partnership with Hampshire Equipment Services to ensure people have access to the right equipment to support their recovery and independence.
