Why our reablement teams love supporting people to regain independence
Reablement is a vital in-house, multi-disciplinary service that delivers short-term support, enabling people to return home safely after a hospital stay and begin their recovery. It also supports those at risk of hospital admission due to sudden changes in their circumstances, helping them maintain or regain independence.
We use the diverse skills and expertise of our workforce to support individuals in achieving their personal goals. This includes Occupational Therapists, Sensory Services Officers, Community Response teams, Business Support and we work closely with Hampshire Equipment Services.
With Reablement hubs located across North, South, West and Central Hampshire, we offer a countywide service to residents, promoting independence and wellbeing at every stage.
Our Rapid Response service is available from 7am to 10pm, every day of the year, enabling us to deliver flexible, timely support whenever it’s needed most.
