Evidence of eligibility needed for a Gateway Card or Gateway Card+

Evidence we can accept
Please ensure that any evidence you provide includes the parent(s)/carer(s) name and address, as well as the child’s name.
We can accept letters from the following:
- the family’s GP
- a Hospital Consultant
- a Diagnostic Assessment Service
- a Paediatrician or Health Visitor
- the child’s Teacher or SENCo
- a Social Worker or Professional from CAHMS involved with the family
Education, Health and Care Plan (EHCP)
We can also accept a full EHCP. This needs to be the whole document of your child’s original EHCP, not an annual review or draft EHCP.

Disability Living Allowance (DLA) and Personal Independence Payment (PIP)
We will accept a receipt of DLA or PIP.
This should be your most recent award letter, showing the rates awarded and how long for.
Evidence we cannot accept
Examples of evidence that we cannot accept include:
- bank statements showing a DLA or PIP payment
- DLA or PIP award letters that have expired
- an annual review or draft of your child’s EHCP
- a copy of your child’s previous Gateway Card or Gateway Card+
If you do not have evidence of eligibility, a template has been produced for the professionals you are in contact with to complete. This can be used if you do not already have a letter confirming your child's needs.
