1. DECLARATIONS OF INTEREST

3. CHAIRMAN'S COMMUNICATIONS
To receive any communications the Chairman may have for this meeting.
4. APPOINTMENT OF CO-OPTED MEMBER
To consider the appointment of Councillor P. Luffman as a Co-opted Member of the
Committee who has been nominated by the Hampshire and Isle of Wight Local
Government Association.
5. COUNTESS MOUNTBATTEN HOSPICE
To receive a presentation from Southampton University Hospitals Trust on proposals to close beds at the Hospice
6. CARE AT THE END OF LIFE
To receive information on the progress being made on the recommendations made by the Care at the End of Life Review to include:
· Confirmation of PCT spending levels on end of life care
· Progress with mapping patient pathways
· Steps to identify and address variations in service delivery and improve consistency in access
· Action to co-ordinate services across different care settings, including OOH services
· Confirmation of the pattern of round the clock community cover across Hampshire
· Action taken to improve access to specialist equipment
· Up-date on the roll out of Care at the End of Life training programmes
· Progress with seeking the views of carers
