If you need further help with social care issues, adult social care have trained and skilled staff who can assist you.

How we will communicate with you

For any written information we need to send to you, we will use email rather than post.

If you are unable to access emails or you have specific communication needs relating to a disability, impairment or sensory loss, we will use your preferred method of communication.

Making referrals and requests to adult social care

Making a request for your own care and support

We will need to know:

  • your GP details 

  • about how you're coping at home

  • your nutrition; can you prepare your own meals, are you having enough to eat and drink?

  • how you manage your personal hygiene; how easy it is to have a wash?

  • how you manage your toileting needs; is your bathroom accessible?

  • if you are wearing appropriate clothes; are you warm enough in winter, are your clothes easy to put on? 

  • how safe you are at home - is your house clear enough to move around? Do you have steps in or outside your home? Is your house warm enough? 

  • who else can help you; your family, a neighbour or friend 

  • if you can get out and about 

  • if you care for anyone else 

  • if you need any equipment. We do not issue walking aids (for example sticks or frames), wheelchairs or pressure relief (for example cushions). If your query relates to these issues, please contact your GP surgery

  • if you think you are being abused or harmed in any way; we refer to this as safeguarding

  • if you have a query about an invoice or payment

Making a request for someone else

If you are providing care for someone else, you might find our information about being a carer useful.

We will need to know:

  • your details

  • the details of the person you are contacting us about

  • their GP details

  • how they're coping at home

  • if they can prepare their own meals; are they having enough to eat and drink, do they purchase meals from a delivery service?

Let us know the kind of abuse you think has taken place. Let us know what other organisations you have raised your concern with, for example, the police – as a professional, you need to ensure you have raised any concerns with the appropriate organisation before making a referral to us

If you need to, you can contact Hampshire County Councils adult social care team on the following phone number. Due to high demands on our service, telephone wait times can be very long, and we would recommend you contact us using one of the electronic forms above.

You can complete our Needs Checker form to get an indication of whether you are likely to be eligible for care and support from us.

If you have not heard from us, and your situation has not changed, you do not need to contact us again. We review and prioritise all requests as we receive them and will be in touch as soon as we can.
