Mental health teams

Social Care Mental Health Teams work together with the Health Service's Community Mental Health Teams and deal with people's social care needs alongside their health needs.

They help people in the community, whether they have just become ill, have a short-term illness, or need on-going support (for example, if they have a long-term illness, or after they have been discharged from a psychiatric hospital).

If you live in the North East of Hampshire the mental health services are provided alongside Surrey and Borders Partnership NHS Foundation Trust.

For the rest of Hampshire services are provided alongside Hampshire and Isle of Wight Healthcare NHS Foundation Trust.

If you live in Southampton or Portsmouth the mental health services are provided by your local unitary authority, not by Hampshire County Council:
