Early Years Transition Service
The Early Years Transition Service is part of the Services for Young Children (SfYC) Inclusion team. The service will support children transitioning to reception year (Year R) at school that have been identified as having a need at SEN support level, and their needs may escalate during the transition to school.
SEN support is additional or different help provided in early years or schools to children with special educational needs or disabilities (SEND), to help them access education and achieve learning objectives.
By maximising strengths based, person-centred approaches, the service supports improved outcomes for children and young people with SEND. The team works alongside early years settings, schools, and parents to build confidence and capability, enabling children to transition from Early Years settings into mainstream Year R without an escalation in support needs, using the Portage Model and Portage Principles.

Support will be offered to children during the summer term in their pre-school, nursery or childminder setting and throughout the transition and into Year R in school.
Support visits will be offered to the Early Years setting, and a transition practitioner will support the planning of the transition meeting.
Support will be offered to the Year R team at the child’s school during the transition and in the autumn and spring terms. (Hampshire schools only).
Support visits to school will typically be offered fortnightly.
A transition partnership agreement (TPA) or appropriate transition meeting paperwork must be written, and a meeting with the receiving Hampshire school arranged.
Support for parents: parents can access a range of webinars and can contact the team via the general enquiry link in the contact section.
Support will be offered to children whose needs can be met at SEND support level through ordinarily available provision and who have not applied for an Education, Health and Care (EHC) plan.
