Contract management
The Older Adults Commissioning Team will be completing regular Contract Management visits and calls with providers a minimum of once a year. Where Hampshire County Council commission over 50% of a provider’s bed capacity, you are likely to receive a minimum of 2 visits per year. These visits will take place face to face.
Providers will be asked to report on areas of activity via a Self-Assessment form which will be distributed twice yearly.
Resident & friends/family survey feedback should be attached as part of the self-assessment along with case studies to showcase your fantastic work. Responses can be discussed during the Contract Management meeting where we will also discuss your performance data which will be shared with you prior to the meeting.
How do I know what services I am meant to be delivering?

What does the provider self-assessment form look like?
The provider self-assessment form will be sent to you automatically 4 weeks before your scheduled visit. You will have 2 weeks in which to fill in and return your form. You can see an example of what the form looks like and the type of questions it will ask.
