Important notice

Please be aware that any care home placements that commence from May 13th will be under Hampshire County Council’s Care Home Framework. If you have been advised to look at this site before this date it will be because the date a placement begins will be on or after May 13th, and therefore the information on this site is relevant and important to you/your loved one.

Contract management

The Older Adults Commissioning Team will be completing regular Contract Management visits and calls with providers a minimum of once a year. Where Hampshire County Council commission over 50% of a provider’s bed capacity, you are likely to receive a minimum of 2 visits per year. These visits will take place face to face.

Providers will be asked to report on areas of activity via a Self-Assessment form which will be distributed twice yearly.

Resident & friends/family survey feedback should be attached as part of the self-assessment along with case studies to showcase your fantastic work. Responses can be discussed during the Contract Management meeting where we will also discuss your performance data which will be shared with you prior to the meeting.

The services you should be delivering are outlined in your service specification.

The provider self-assessment form will be sent to you automatically 4 weeks before your scheduled visit. You will have 2 weeks in which to fill in and return your form. You can see an example of what the form looks like and the type of questions it will ask.

We will always try to ensure we have spelt out an acronym before we use it any further within a document. The CQC provides a useful glossary of terms for providers.

If you are still unsure please contact [email protected].

The Quality Outcomes and Contract Management Framework explains the steps Hampshire County Council’s Adult’s Health and Care will take to ensure care and support services in Hampshire are reviewed and monitored to deliver outcome focused person centred care.
