Community Response Teams

What we do

Our Community Response Teams (CRT) are part of the multi-disciplinary Short Term Reablement Service. They are CQC registered reablement domiciliary care teams that provide the regulated activity of personal care.

CRT can support you by assessing and assisting you with everyday activities such as personal care, washing and dressing. The team will also help to ensure you are able to manage your food and drink. If we assess that your recovery would be helped by therapy-based exercises and routines, the team can also support you with this.

How to access this service

CRT only accept referrals from hospital and community based social work teams. They are unable to deal with direct enquiries regarding referrals.

If you are in hospital, the social work team will make a referral if they feel that this service would be right for you, so you do not need to contact us.

If you are at home and would like an assessment of your needs, please contact Adult Services who will discuss your situation with you and make a referral to CRT if suitable.
