Prevention - staying well
In this digital age making sure people can find reliable information and advice that will help them to stay well and maintain their independence is more important than ever.
Staying well requires a proactive approach to health, where individuals are equipped with the knowledge and resources to make informed decisions about their wellbeing.
Encouraging people to stay well is important, helping people do that in a way that suits them, their lifestyle and those around them matters to us.
Here are some examples of the resources available to the residents of Hampshire, how people benefit, and the number of people being helped.
Helping you to make informed choices by ...
providing the information and advice that will help people to stay well and independent
Connect to Support Hampshire website had over 367,000 visits in the last year by residents looking for information and advice to help them look after themselves, stay safe and connect with the local community.
With resources available, including support and advice for unpaid carers, information about local community services, support and care options, and advice on keeping well and staying safe it is a valuable resource for anyone who needs information and advice.
240 Connect to Support Champions help to increase awareness of this great website and printed guide. If you would like to help us to get the word out contact [email protected].
It's a good, well-presented site and the content is interesting. My mum has dementia, so I check in on the Connect to Support Hampshire site every now and then to see what's new. I always learn something interesting and helpful.
Helping you understand your care needs by ...
giving you the tools to do so in your own time.
Over 120,000 people contact Hampshire County Council every year to find out if they are eligible for care services - this is often the first time a person will contact their local authority about Adult Social Care services.
To help people get the information and advice they need without waiting to talk to someone, Hampshire County Council has on-line self-service tools called needs checkers.
Developed with the support of the Citizen’s Advice Bureau, needs checkers enable potential service users and carers to check their eligibility for support and find the necessary information at their convenience.
Over the last few years needs checkers have been used by residents over 4500 times and have resulted in a reduction in people contacting Hampshire County Council for support and an increase in information and advice being given through Connect to Support Hampshire.
Encouraging you to stay fit by ...
funding projects and initiatives that help people to be active and stay in touch with their local community.

The services come with the added benefit of a wellbeing check, with delivery drivers trained to spot customers who may need extra support.

Safeguarding
A key part of prevention in social care.
Safeguarding is a cornerstone of our commitment to providing high-quality adult social care. Safeguarding is not just about responding to incidents of harm or abuse; it is fundamentally about prevention. By creating a safe and supportive environment, we can proactively protect vulnerable adults from harm, ensuring their well-being and dignity.
Our approach to safeguarding involves a comprehensive strategy that includes raising awareness, training staff, and fostering a culture of vigilance and respect. We believe that prevention is the most effective form of protection. By identifying risks early and taking appropriate actions, we can prevent harm before it occurs.
Through collaboration with local partners, continuous improvement of our practices, and active engagement with the community, we strive to create a safe and inclusive environment for all. Safeguarding is everyone’s responsibility, and together, we can make a significant difference in the lives of those we support.
Feeling safe and secure in your own home is fundamental to well-being. Connect to Support Hampshire offers valuable information and advice on available support and resources, including self and well assessments by the Hampshire and Isle of Wight Fire Service, which help identify potential risks in your home and provide guidance on necessary actions. See Staying safe in your home for more information.
You can help - everyone has a role to play in safeguarding, understanding what it is and how to spot the tell-tail signs are important. If you are concerned that you or another adult is being neglected, harmed or abused in any way, do not ignore it.
Safe Place Scheme
Our Safe Place scheme is for vulnerable people and people with learning disabilities, providing a safe place for people feeling scared or upset when they are out on their own. You can help if you have a business or shop that could be a safe place.
